Job Description:

Provide Contact Center-based support via phone, email, fax, and web to facilitate placement requests for patients choosing DaVita visiting or permanent dialysis. Use DGS tools and resources to coordinate placements, contribute to CRM knowledge databases, and suggest process improvements. Uphold DaVita's values and ensure exceptional customer service. This role also supports emergency dialysis placements and collects feedback from patients and partners.

Roles and Responsibilities:

• Respond professionally to inbound calls, emails, and faxed referrals
• Request placements and respond to treatment option inquiries
• Make outbound calls to hospitals to complete placement requests
• Collaborate with Field Operations to build hospital/doctor relationships
• Meet performance metrics and call quality standards
• Use tools/resources to complete patient placements
• Maintain and enter data forms received from the field
• Support process development and enhancement
• Build strong relationships with hospitals, discharge planners, and internal teams
• Work during non-traditional hours across time zones
• Ensure HIPAA compliance and protect PHI
• Provide high-quality, empathetic customer service under pressure
• Maintain organized, accurate, and detailed work
• Proactively contribute to CRM and process improvements
• Work overtime as needed
• Follow DGS and DaVita policies and procedures
• Ensure regular and punctual attendance
• Collaborate with DaVita teammates across locations
• Perform additional duties as assigned

Required Skills:

• High School Diploma or equivalent
• Minimum 6 months of related experience
• Data entry speed: 25-40 KSPM
• Basic skills in Microsoft Excel and Access
• Intermediate proficiency in Microsoft Word and Outlook

• Understanding of kidney disease and dialysis treatment options (In-Center, Self-Care, Nocturnal)
